Issuance of Certification, Authentication and Verification (CAV) of Academic Records for Private and Locally Funded Colleges and Universities

Clients

• College students and graduates thru HEI representative
• Individual college student and graduate

Requirements:

A. For Graduates of Bachelors Degree/Graduate and Post-Graduate Degree
1. Indorsement from the HEI Registrar
2. Copy of Transcript of Records (TOR) and/or Diploma and other records duly certified by the
School Registrar
3. Certified photocopy of Special Order (SO)

B. For College Students/Undergraduates

1. Indorsement from the HEI Registrar
2. Certification of Enrolment
3. Copy of Transcript of Records (TOR)/earned units certified by the School Registrar

Total/Maximum Duration of Process:
Abroad:10 -15 working days
Local: 5 working days

How to Avail of the Service

Step 1:Submits required documents they will receive and verify/validate the documents submitted and certification of Registrar, and issues order of paymentReceiving Staff : CHEDRO (Administrative Division)2 minutes per application
Step 2:Proceeds to the Cashier for payment of CAV Fee (Issues Official Receipt) (OR) from the Cashier/Cash Clerk CHEDRO (Administrative Division)2 minutes
Step 3:Submits accomplished form and OR and receive claim stubIssues Claim Stub to be presented to the DFA for issuance of the authenticated documents with Red Ribbon or CHEDRO, for local purposesReceiving Staff CHEDRO (Administrative Division)5 minutes
Step 4:Processes the submitted documents: 20 minutes
  1. Prepares the CAV for signature of the Chief Administrative Officer
  2. Stamps the document with dry seal
  3. Releases the CAV and files the second copy of the
Step 5:Claims the CAV (local employment) at CHEDRO 3 minutes
Step 6:CAV for employment abroad: 30 minutes
  1. Prepares masterlist and transmits application received through DFA’s official courier for DFA authentication on a daily basis.
